29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A). On or about 09/15/16, at the site of 39-54 222nd Street. Bayside, NY   Employees working at the site were in danger of head injury from falling objects such as, but not limited to tools and bricks from other employees who bricklaying the exterior wall on the frame scaffolds. The employees were not provided protected helmets.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

29 CFR 1926.451(g)(4)(iv): Midrails were not installed at a height approximately midway between the top edge of the guardrail system and the platform surface:    A).      on or about 09/15/16, at the site of 39-54 222nd Street. Bayside, NY    Employees working on frame scaffolds were exposed to the hazard of falling approximately 18 ft. to the ground. The midrails of the scaffolds were missing.    ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
