29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees. (a) Jobsite located near the intersection of Athens Road and Downs Avenue, Mendenhall, MS - On or about September 28, 2016, employees were in an excavation that was 12 feet deep and there was not a ladder or other safe means of egress from the excavation. The employees were riding on the bucket of a Caterpillar excavator to exit the excavation.

29 CFR 1926.651(e): At this job site employees were not prohibited from being underneath loads handled by lifting or digging equipment in that: (a) Jobsite located near the intersection of Athens Road and Downs Avenue, Mendenhall, MS - On or about September 28, 2016, employees were exposed to struck-by and caught-under hazards while working in an excavation under a Caterpillar excavator as it was lowering 8 inch sewer pipe into the excavation.
