The employee was not using any form of fall protection and was exposed to a fall hazard of greater than 25 feet from the roof's eve to the ground below.

The employer had not trained each employee to recognize the hazards of falling and the procedures to follow to minimize the hazards.   NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
