29 CFR 1910.134(c)(1): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use: (Construction Reference 1926.103)  12372 Park Street, Palmer, MA: On or about September 29, 2016 employees were required to wear SCBA respirators in the execution of their work duties and the employer had not developed and implemented a written respiratory protection plan with work site specific procedures and elements for the required respirator use.   Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).

29 CFR 1926.1203(d)(1): The employer had employees it directs enter a permit space and did not have a written permit space program that complies with �1926.1204 at the construction site.   1237 Park Street, Palmer, MA: On or about September 29, 2016 employees were required to enter permit required confined spaces in the form of single walled steel, underground gasoline fuel storage tanks which had been recently excavated in order to replace them with new double walled steel tanks.  Employees were required to enter these excavated tanks in order to clean and sanitize them of remaining hydrocarbons and petroleum distillates in preparation for disposal.   During this operation a written program that complies to 1926.1204 was not available prior to or during entry operations for inspection by affected employees and their representatives.  Specific abatement documentation that this violation has been corrected must be provided within 10 days of the abatement in accordance with 29 CFR 1903.19(d)(1).
