Employees are tasked with servicing, maintaining, or setting up equipment such as but not limited to press brakes, shears, benders, and gorelockers. Hazardous energies include but are not limited to mechanical, electrical, pneumatic, gravitational, and hydraulic.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On or about September 19, 2016, the employer had not performed a periodic inspection of the energy control procedures within the last year in which maintenance, servicing, or set up of equipment had taken place. The company had in place a written LO/TO program, however machine specific procedures were not in place.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On or about September 19, 2016, the employer failed to ensure that the Roto-Die press brake was effectively guarded to prevent employee contact with the hazardous area of the dies closing. Barrier guards and other devices were not in place on both the front and rear of the equipment. The mechanical action of the press brake is controlled by foot pedal allowing the operator's hands to reach the hazard area. b) On or about September 19, 2016, the employer failed to ensure that the Drossbach SGB 1250 Gorelocker was effectively guarded to prevent employees from contacting the pinch points of the rollers. The rotation of the rollers is controlled by foot pedal allowing the employee's hands to reach the hazard area during operation. c) On or about September 19, 2016, the employer failed to ensure that the LockFormer Transverse Duct Connector was effectively guarded to prevent employees from the pinch points of the rollers. The barrier guard was not in place on the equipment while employees operated it.
