OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: On or about September 27, 2016, employees re-dressing tool joints at the Kaiser Well #1, located at SEC 13, TWP 19S, R 13W, Great Bend, KS, were exposed to struck-by hazards created by a swinging chain/hook attached to the bushing puller which was being rotated by the drilling table. Among other methods, some feasible and acceptable abatement methods to correct this hazard may include but are not limited to the following: 1. Allow no personnel to clean, lubricate or repair any machinery where there is a hazard of contact with moving parts until such machinery has been stopped or such parts have been properly guarded as indicated in Paragraph 6.8.5 of the American Petroleum Institute Recommended Practice 54. 2. Develop, implement and enforce good written procedures that ensure all qualified operators can correctly and safely perform their assigned duties as indicated in Paragraph 3.1.1 of American Petroleum Institute Publication 770.

29 CFR 1910.23(a)(9): Every floor hole into which persons cannot accidentally walk (on account of fixed machinery, equipment, or walls) were not protected by a cover that leaves no openings more than one inch wide: At the Kaiser Well #1, located at SEC 13, TWP 19S, R13W, Great Bend, KS, there were two floor holes measuring approximately two inches wide in the grated walkway along the side of the doghouse next to the drill floor.
