29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees: (a) Packaging Lines - Production employees removed guards, which enclosed moving machine parts such as chain drives and rotating shafts, near floor level on bagger machines, in order to clean crumbs and seeds from this area. The employees turned the machinery off but did not apply lockout/tagout when doing this cleaning task, exposing employees to getting caught in moving machine parts, on or about August 30, 2016. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: (a) Packaging Lines - The lockout/tagout training provided to employees by the employer did not ensure that employees understood the purpose and function of the energy control program in that production employees who worked the Baggers did not understand that the use of complimentary equipment, such as a compressed air nozzle or hand-held brushes, with machinery turned off but not locked out/tagged out and with guards removed, did not provide protection against energy hazards during machine cleaning (classified as a servicing and maintenance activity), on or about August 30, 2016. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
