29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  On or about 10/03/16, at the jobsite located at 1106 Wildhorse Meadows Drive, Chesterfield, MO  63005, employees were applying shingles using pneumatic nail guns without donning the necessary eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 10/03/16, at the jobsite at 1106 Wildhorse Meadows Drive, Chesterfield, MO  63005, employee(s) engaged in residential roofing activities were exposed to a fall distance of approximately 17 feet, 9 inches - 20 feet, 10 inches at the rear of the residential structure and approximately 21 feet, 5 inches at the front of the residential structure as they worked without any form of fall protection on an approximate 8:12 pitch roof.

29 CFR 1926.503(a)(1): The employer does not ensure that  a training program for each employee who might be exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    The employer failed to adequately train workers performing residential roofing activities on fall protection requirements when working on an approximate 8:12 pitch roof.
