29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers was not equipped with standard stair railings or standard handrails as specified in paragraphs (d)(1)(i) through (v) of this section, the width of the stair to be measured clear of all obstructions except handrails: a) At the facility, inside the box room, on or about October 03, 2016, and at times prior thereto, the employer exposed employees to trip hazards and falls of up to 5 feet to the concrete floor, in that a 5-riser stairway on the Thiele Technologies box machine was not provided with adequate standard railings as employees accessed and egressed the box machine platform.
