The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection. This most recently occurred on October 3, 2016 on the jobsite located at 20815 Sequoia St, Elkhorn, NE 68022. Employees were observed reroofing a residence on a 10:12 to 14:12 pitched roof at heights greater than 6 feet above the ground below.

29 CFR 1926.1053(b)(9): The area around the top and/or bottom of (a) ladder(s) was not kept clear. The employer is failing to ensure that all employees are protected from fall and trip hazards when entering/exiting the landing surface of a residential roof. This most recently occurred on October 3, 2016 on the jobsite located at 20815 Sequoia St, Elkhorn, NE 68022. Employees were observed using ladders that had debris on and around the landing surfaces. The top landing surface was approximately 14 feet high.
