The intermediate rail was not half way between the top rail and the floor, platform, runway, or ramp:    On or about October 3rd, 2016, employees were exposed to a fall hazard on mezzanine platforms where fall protection did not meet proper specifications to guard against the hazard.    a. Mezzanine location 1.45 - The top railing chain was at approximately 27 inches above the floor at its center and the intermediate railing chain was approximately at 8 inches above the floor at its center.     b. Mezzanine location 1.30 - The top railing chain was connected at the intermediate railing chain connection point.     c. Mezzanines location 1.30 - The stairway landing did not have an intermediate railing.
