29 CFR 1910.23(e)(1): A standard railing did not consist of a top rail, intermediate rail, and post, and did not have a vertical height of 42 inches nominal from upper surface of top rail to floor. The intermediate rail was not half way between the top rail and the floor, platform, runway, or ramp: On or about October 3rd, 2016, employees were exposed to a fall hazard on mezzanine platforms where fall protection did not meet proper specifications to guard against the hazard. a. Mezzanine location 1.45 - The top railing chain was at approximately 27 inches above the floor at its center and the intermediate railing chain was approximately at 8 inches above the floor at its center. b. Mezzanine location 1.30 - The top railing chain was connected at the intermediate railing chain connection point. c. Mezzanines location 1.30 - The stairway landing did not have an intermediate railing.
