29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     (a)  Jobsite:  On or about October 3, 2016, four employees were not protected from falling to the ground while they were doing roofing work.  Two employees were installing guardrail brackets and 2 employees were installing roof sheeting.   In addition, there were openings of about 6 feet in the guardrail on the eave side and about 8 feet of opening on the rake side of the roof.  The fall hazard to the four employees was about 33 feet to he ground.      (b) Jobsite:  On or about October 3, 2016, an employee was walking the trusses and he was exposed to a fall hazard of between 12 and 18 feet.

29 CFR 1926.503(c): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer is to retrain each such employee such as inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicate that the employee has not retained the requisite understanding of skill:     (a) Jobsite:  On or about October 3, 2016, there were four employees that were doing roofing work and they need to be retrained on how and when to use fall protection when doing roofing work.
