29 CFR 1926.50(c): A person who has a valid certificate in first-aid training was not available at the worksite to render first-aid: (a) Hamlin, Residential Roof: On or about October 3, 2016 and times prior, there were no infirmaries, clinics or hospitals in near proximity nor certified personnel trained to render first aid in the event of an employee injury.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Hamlin, Residential Roof: On or about October 3, 2016 and times prior, an employee was removing old roofing materials on a roof that measured approximately 8.5 feet from the eave to the ground and did not have any fall protection systems in place. Walter Conley dba Conley Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1073465, citation number 1, item number 1a and was affirmed as a final order on July 25, 2015, with respect to a workplace located at Sapphire Street Hurricane, WV 25526.
