29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards: 29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: APM Holdings LLC work site located at 2124 Kohler Memorial Dr. Sheboygan, WI: On and before 9/26/16, employees installing computer related equipment were exposed to fall hazards when they had not been trained to recognize and avoid hazards relating to the use of portable ladders.

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, within (24) twenty-four hours. APM Holdings LLC project site located at 2124 Kohler Memorial Dr. Sheboygan, WI: On 26 September, 2016, an employee was hospitalized following a fall from a ladder and the employer's representative did not report the incident until more than 48 hours later.
