29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: a) 5170 N. Northwest Highway, near Mezzanine- On October 3, 2016, the employer did not ensure that flexible cords were not used as a substitute for fixed wiring. A power strip was used to supply power to a water fountain. b) 5190 N. Northwest Highway, Break Room and Back Area- On October 3, 2016, the employer did not ensure that extension cords were not used as a substitute for fixed wiring. Extension cords were used to supply power to overhead lamps, a vending machine, and a parts oven located in the outside back area, as well as another extension cord.

Northwest Highway, southeast corner of building- On October 3, 2016, the employer did not ensure that the exit route lead to an open space with access to the outside. The exit route in the southeast corner of the building leads to a courtyard with a dead end and is cluttered with material, including scrap metal.
