29 CFR 1910.134(c)(2)(i): An employer may provide respirators at the request of employees or permit employees to use their own respirators, if the employer determines that such respirator use will not in itself create a hazard. If the employer determines that any voluntary respirator use is permissible, the employer shall provide the respirator users with the information contained in Appendix D to this section ("Information for Employees Using Respirators When Not Required Under the Standard");   a) In the production area - Employees who use respirators such as, but not limited to, Uline Surgical Masks Model S-10478 and Body Guard Safety Gear by Honeywell Dust Masks were not given Appendix D on or about 1/12/17.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, safety data sheets, and employee information and training will be met, and which also includes the following:  a) Production Area- Employees use and are exposed to liquid petroleum gas (LPG) when changing LPG tanks on forktrucks without being provided with a written Hazard Communication program on or about 1/12/17.  b) Throughout the facility - Employees use and are exposed to chemicals such as, but not limited to, cleaning chemicals , inks, and adhesives when cleaning areas such as, but not limited to floors, restrooms, and production equipment and working in the ink department, respectively without being provided with a written Hazard Communication program on or about 1/12/17.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.1200(g)(8):   The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical, and did not ensure that they were readily accessible during each work  shift to employees when they were in their work area(s)  a) Throughout the facility - Employees use and are exposed to chemicals such as, but not limited to liquid petroleum gas (LPG), multiple cleaning chemicals, inks, and adhesives when changing LPG tanks on forktrucks, cleaning, and working in the ink department, respectively. Employees were not provided with safety data sheets for all chemicals in which they are exposed to on or about 1/12/17.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  a) Throughout the facility - Employees use and are exposed to chemicals such as, but not limited to, liquid petroleum gas (LPG), cleaning chemicals , inks, and adhesives when changing LPG tanks on forktrucks, cleaning areas such as, but not limited to floors, restrooms, and production equipment and working in the ink department, respectively. Employees have not received updated hazard communication training on or about 1/12/17.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
