29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (Construction Reference: 1926.59).  On or about 10/4/2016, Team Environmental Group employees working at the Department of Veterans Affairs Medical Center located at 3400 Lebanon Pike in Murfreesboro, TN were exposed to hazardous materials (eye and skin irritants) for which the employer did not maintain and make readily available a copy of the written hazard communication plan in the work place.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.1200(e)(1)(i): The employer did not compile a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet.  On or about 10/4/2016, Team Environmental Group employees working at the Department of Veterans Affairs Medical Center located at 3400 Lebanon Pike in Murfreesboro, TN were exposed to hazardous materials (eye and skin irritants) for which the employer did not compile a complete chemical list of all hazardous materials used in the work place.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

On or about 10/4/2016, Team Environmental Group employees working at the Department of Veterans Affairs Medical Center located at 3400 Lebanon Pike in Murfreesboro, TN were exposed to hazardous materials (eye and skin irritants) for which employees did not know how to access copies of safety data sheets for the hazardous materials they use.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.134(e)(6)(i)(C): The employer did not obtain a statement that the physician or other licensed health care professional (PLHCP)  had provided the employee with a copy of the PLHCP's written recommendation: (Construction Reference 1926.103).   On or about 10/4/2016, Team Environmental Group employees working at the Department of Veterans Affairs Medical Center located at 3400 Lebanon Pike in Murfreesboro, TN were exposed to asbestos containing materials for which they wore North 7700 half face air purifying respirators and the medical clearance forms did not include a   statement that the physician or other licensed health care professional (PLHCP)  had provided the employee with a copy of the PLHCP's written recommendation.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
