29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A). On or about 09/20/16, at the site of 168-14 81st Ave. Jamaica, NY   Employees working on the ground were in danger of head injury from falling objects such as, but not limited to tools and material from the employees who installing siding on the steep roof. The employees were not provided protected helmets.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:  a) On or about 09/20/16, at the site of 168-14 81st Ave. Jamaica, NY  Employees working on the lower steep roof near by the power lines was exposed to electrical shock hazards.  ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.

ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

Employees were exposed to fall hazards of 11-feet while working and were not provided with fall protection.      ABATEMENT DUCUMENTATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being access:    a)        on or about 09/20/17, at 168-14 81st Ave. Jamaica, NY    Employees were using a portable ladder to gain access to a lower steep roof. The ladder side rails extended less than one foot above the upper landing surface was secured. The employees were exposed to the hazard of falling approximately 11 ft. to the ground below.   ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19.
