29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:  a. 264-268 US 28 (South Ave.) Fanwood, NJ  Employees engaged in framing activities of a commercial building were observed working from a welded tubular frame scaffold that was not inspected by a competent person before each work shift.  Defects observed included, but were not limited to, the scaffold not fully planked between the front uprights and aft guardrail supports on the working levels, scaffold frames missing base plates, scaffold frames supported by unstable objects, scaffold being erected within 3 feet of a 440v power drop, and scaffolds not having safe access to work platforms.   Violation observed on or about October 4, 2016

29 CFR 1926.451(b)(1): Platform(s) on all working levels of scaffolds, were not fully planked or decked between the front uprights and guardrail supports:     a. 264-268 US 28 (South Ave.) Fanwood, NJ    Employees engaged in framing activities of a commercial building were observed working from a welded tubular frame scaffold that was not fully planked between the front uprights and aft guardrail supports on all working levels.  Employees installing sheathing worked from a welded tubular frame scaffold that had single planking on several levels as a work platform.     Violation observed on or about October 4, 2016

29 CFR 1926.451(c)(2)(ii): Unstable objects shall not be used to support scaffolds or platform units:   a. 264-268 US 28 (South Ave.) Fanwood, NJ  Employees engaged in commercial framing activities were observed working from a welded tubular frame scaffold that was supported by unstable objects.  The employer authorized the use of scrap wood (2x4) to support scaffold leg when the concrete under the pad was excavated for the installation of new building footings.  Violation observed October 4, 2016

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  a. 264-268 US 28 (South Ave.) Fanwood, NJ  The employer did not ensure safe access to the scaffold platforms when scaffold platforms were more than 2 feet above the point of access.  Employees engaged in commercial framing activities used the scaffold cross braces as access to the working platforms.    Violation observed on or about October 4, 2016  b. 264-268 US 28 (South Ave.) Fanwood, NJ  The employer did not ensure safe access to the scaffold platforms when scaffold platforms were more than 2 feet below the point of access.  Employees engaged in commercial framing activities were observed climbing over a parapet wall and onto a single plank work platform of a welded tubular frame scaffold. Employees engaged in commercial framing activities worked directly next to an energized power drop and were observed egressing the roof by climbing over the conductor to access a scaffold platform. 29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a. 264-268 US 28 (South Ave.) Fanwood, NJ  The employer did not ensure employees engaged in commercial framing activities working at unprotected working surfaces greater than 6-foot above the next lower level were protected from falls.  Employees removing construction debris from the building roof were observed walking over to the outside edge to drop the debris into a dumpster and were not protected from falls by use of a guardrail system or a personal fall arrest system.  Violation observed on or about October 4, 2016
