29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o). a) Control Room Entrance, 1500 Purdytown Turnpike, Hawley, PA - The unguarded pulleys for the asphaltic oil pump that were thirty two-inches or less (<=32") above ground exposed employees working near the asphaltic oil pump and employees that entered/exited the control room to caught-in hazards (ingoing nip points and sheave spokes), as observed on or about October 5, 2016. b) Dust Auger, 1500 Purdytown Turnpike, Hawley, PA - The unguarded pulleys for the dust auger that were six-inches (6") above ground level exposed employees that worked on and/or near the dust auger to caught-in hazards (ingoing nip points), as observed on or about October 5, 2016.

29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs 42 inches or less from the floor level were not fully enclosed by guards conforming to requirements specified in 29 CFR 1910.219(m) and (o). a) Control Room Entrance, 1500 Purdytown Turnpike, Hawley, PA - Both runs of the three (3) unguarded asphaltic oil pump horizontal belts were less than forty two-inches (<42") above ground level and exposed employees working near the asphaltic oil pump and employees that entered/exited the control room to caught-in hazards (ingoing nip points), as observed on or about October 5, 2016. b) Dust Auger, 1500 Purdytown Turnpike, Hawley, PA - Both runs of the two (2) unguarded dust auger horizontal belts were less than forty two-inches (<42") above ground level and exposed employees working on and/or near the dust auger to caught-in hazards (ingoing nip points), as observed on or about October 5, 2016.

29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check. a) Asphaltic Oil Tank, 1500 Purdytown Turnpike, Hawley, PA - An Amerex ABC Multi-Purpose Stored Pressure Dry Chemical Extinguisher Model S9873 had not had an annual maintenance check since been inspected since 2012 exposing employees who choose to attempt to extinguish a fire with this portable fire extinguisher to fire hazards, on or about October 5, 2016.
