29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift. On the south side of the construction site, employees using a 860SJ JLG lift to access and egress the roof without being secured to the basket were exposed to fall hazards approximately 34 feet to the ground below.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On the south side of commercial building, employees installing metal flashing from a 10:12 pitch roof without fall protection were exposed to a fall hazard of approximately 34 feet to the lower level.
