29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  (a) On or about October 4, 2016 at worksite 1101 SW Conch Way, Blue Springs, Missouri- employees were exposed to serious eye injury while performing roofing operations using a Bostitch brand pneumatic nail gun without eye protection.  Abatement certification not needed for this item.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   FRANCISCO DELAROSA ROOFING was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(13), which was contained in OSHA inspection number 922483, citation number 01, item number 01 and was  affirmed as a final order on 08/15/2013, with respect to a workplace located at 123 Nw Morgan Street, Platte City, MO 64079.  (a) On or about October 4, 2016 at worksite 1101 SW Conch Way, Blue Springs, Missouri- employees were performing  roofing operations at an elevated height of approximately 21-27 feet without any means of fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems.  Abatement certification is not required for this item.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    The Francisco Delarosa Roofing was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1) , which was contained in OSHA inspection number 922483, citation number 01, item number 02 and was affirmed as a final order on 08/15/2016 with respect to a workplace located at 123 NW Morgan Street in Platte City, Missouri.  (a) On or about October 4, 2016 at worksite 1101 SW Conch Way, Blue Springs, Missouri-. Employees were accessing a working surface on a steep pitch roof by way of a portable ladder whose side rails did not extend at least 3 feet above the upper landing surface being accessed.     Abatement certification for this violation is not required
