Condition observed on or about 10/04/16.     b- 515 Union Avenue, Lakehurst NJ: An employee wearing a full body harness with no lanyard attached, was exposed to fall hazards in excess of 10 feet to the ground surface below when performing solar panel bracket installation and other related activities from the first story roof of a residential structure having a pitch of approximately 6 on 12.  Condition observed on or about 10/04/16.     Vivint Solar, Inc. was previously cited for a Repeat violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1064664, citation number 1, item number 1 and was affirmed as a final order on 07/13/15, with respect to a workplace located at 227 Williams Street, Hasbrouck Heights NJ.    Vivint Solar, Inc. was previously cited for a Serious violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 758441, citation number 1, item number 1 and was affirmed as a final order on 04/08/13, with respect to a workplace located at 93 Montgomery Street, Jersey City 07302.
