29 CFR 1926.503(c)(3): Fall protection retraining was not done when inadequacies in an affected employee's knowledge or use of fall protection systems or equipment indicated that the employee had not retained the requisite understanding or skill: a) Job site, near 936 Bell Avenue, Wexford, PA: On or about October 4, 2016, during roofing activities, the employer did not ensure that employees were properly retrained where exposed to falls of at least approximately 20 feet. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection systems/devices at all times, connecting restraint systems to the harness hip area D-rings rather than the rear area D rings, having rope grab systems set up/used in such a manner that an employee could fall over 6 feet and be exposed to swing hazards, and failing to utilize a shock absorbing lanyard system in these same instances.
