29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm to employees in that an employee was exposed to crushing hazards.  a) 3400 Lebanon Pike (Murfreesboro, TN) - On or about 10/04/16, an employee was exposed to crushing hazards when not wearing a seat belt while operating a TORO Groundsmaster 360 ride-on mower.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).  The Department of Veterans Affairs was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1960.8(a), which was contained in OSHA inspection number 1145320, notice number 1, item number 1 and was affirmed as a final order on 06/28/2016, with respect to a workplace located at 1200 Bailey Ave Chattanooga, TN 37404.
