29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes (including skylights) by covers:    a). On or about October 5, 2016 employees were cleaning the site and preparing the construction site for incoming hurricane Matthew and were jumping across and working in close proximity to a 5 foot wide 4 feet deep 150 foot long equipment pit in the floor of the concrete slab. The floor hole/pit was not covered by any means of grates, plywood or covers exposing the employees to a slip, trip and fall of 4 feet to the bottom of the pit.
