29 CFR 1910.178(o)(2): Load(s) were being handled which exceeded the rated capacity of the industrial truck(s):    Capstone Logistics forklift operators used the practice of bulldozing pallets when making rows of pallets such as in building 2, between the warehouse dock doors and fixed racking, including pallets stacked two levels high and without a spotter.  Dollar General employees routinely walk in the aisle ways between the rows of pallets potentially being exposed to pallets being moved during the pushing practice.  There was no rated capacity for pallets being bulldozed or other method that determined how many pallets could be bulldozed.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

29 CFR 1910.36(g)(2): Exit access(es) were not at least 28 inches (71.1 cm) wide at all points:    Exit access width was not maintained at least 28 inches such as between rows of product in building 2 warehouse where employees entered the aisle ways in order to check product.  Rows were approximately 70 ft in length and the aisle way least width measured was 16 inches.  Employees were observed walking into aisles of 19 inches and 20 inches.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).
