29 CFR 1910.1025(d)(3)(i): The employer shall monitor employee exposures and shall base initial determinations on the employee exposure monitoring results and any of the following, relevant considerations: Facility Management Maintenance did not conduct initial employee lead exposure monitoring to determine if employees were below the permissible exposure limits (PELs) to lead. Lead was present in paint that was scrapped during window repair and exterior rehabilitation work that was done on buildings at Golden Gate National Recreation Areas by these employees.

29 CFR 1910.305(b)(2)(i): Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: Building 1046: On October 18, 2016, the electric wall switch plate was missing in the bathroom. The electric outlet box was not effectively closed which exposed the employees to potential shock hazard.
