29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded: a) On or about October 5, 2015 - at the job site of 40-24 College Point Blvd, Flushing, NY 11354 Employees were using personal fall arrest systems with lifelines that were tied off to a metal guardrails, with sharp corners, while working on the 15th floor. The lifelines were not protected from being abraded. WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PERSUANT TO 29 CFR 1903.19.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a) On or about October 5, 2015- at the job site of 40-24 College Point Blvd, Flushing, NY 11354 An employee working on the 6th floor of the building was using a personal fall arrest system with a lifeline that was extended approximately 12-feet, laterally, from its anchor point. The employee was exposed to a fall distance 12-feet before his fall would be arrested. WRITTEN ABATEMENT CERTIFICATION IS REQUIRED PERSUANT TO 29 CFR 1903.19.
