29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4):     a) Production Area - The employer failed to develop, utilize and implement a written lockout/tagout program for employees performing repairs and maintenance on equipment such as but not limited to the offset printer, finisher and cutter. The electrical disconnect on the printing press was not locked out prior to an employee attempting to perform repairs and other maintenance on the unit. (CEI digital finisher) (Model # 382)

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:     (a) Production area- The employer failed to develop a written hazard communication program for employees exposed to hazardous substances such as, but not limited to, isopropyl alcohol, Korchem inks and other materials involving the printing operations.

29 CFR 1910.1200(h)(1): Employees were not provided information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard was introduced into their work area.   a) Production Area: The employer failed to provide training for all employees exposed to hazardous chemicals such as, but not limited to isopropyl alcohol and various Kor Chem inks. Employees were exposed to a fire hazard that can and may result in 2nd/3rd degree burns while performing printing operations.
