Milcam, Inc., 660 Fisher Hill St, Sharon, PA 16146, on or about October 5, 2016 and times prior thereto: An employee voluntarily wore a half mask air-purifying respirator (3M brand #6200 series with P100 cartridges) during ceiling tile removal operations at the worksite. The employee was not provided with a medical evaluation prior to the use of the half mask filtering facepiece respirator.
