29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section of the standard: Repair Shop: The employer had not established and implemented written step-by-step hazardous energy control procedures in the machine specific for the employees during servicing and maintenance of equipment, such as (but not limited to) Skyjack Vertical Masts. On or about October 04, 2016, an employee's right index and middle fingers caught-in between the vertical mast and outer cover while working on the Skyjack Vertical Mast Model SJ16 and Serial Number 14004753. The employee sustained crushing finger injuries to tips of both fingers from the incident.
