At the hot asphalt mixing plant, employees were exposed to fall hazards in the following locations:  a.  at the top of the asphalt plant storage silo, a guardrail was removed and an employee was not tied off exposing the employee to an approximately 30 feet fall to the ground below.  b.  at the asphalt plant storage silo, employees accessed the upper level of the silo using fixed ladders that had an unprotected platform exposing employees to an approximately 10 feet fall to the ground below.
