29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: Location: 20 Magnolia Street Employees were not protected against contact with live electrical circuits in walls and above ceilings during demolition activities.

29 CFR 1926.416(a)(3): Before work was begun the employer did not ascertain by inquiry or direct observation, or by instruments, whether any part of an energized electric power circuit, exposed or concealed, was so located that the performance of the work could bring any person, tool, or machine into physical or electrical contact with the electric power circuit; the employer did not post and maintain proper warning signs where such a circuit existed; and the employer did not advise employees of the location of such lines, the hazards involved, and the protective measures to be taken. Location: 20 Magnolia Street, Second Floor The employer did not identify and warn employees about 'live' electrical conductors that had been exposed by demolition work. An employee was shocked when he contacted an exposed conductor.
