29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  JOB SITE:  5212 Shetland Drive, Weldon Spring, MO 63304, on or about 10/6/2016, workers were observed on top a 4:12 pitch roof, approximately 9' 7" to 18' 7" eave to ground level, actively installing new roof shingles and were not protected by guardrail systems, safety net system, or personal fall arrest system.  The workers installed roof jacks with toe boards as the sole means of fall protection.  The workers were exposed to fall hazard.
