Market St., Akron, Ohio 44305, during the installation of insulation material on a commercial construction remodeling project site (McDonald's Restaurant), the employer did not ensure portable ladders were utilized properly by allowing employees to stand on the top step of a 8 foot ladder to gain access to elevated heights.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about October 4, 2016, during the installation of carpentry material at a job site located at 758 E. Market St., Akron, Ohio 44305, the employer did not ensure employees working on a roof (elevated working surface) were protected from a fall of approximately 20.1 feet by utilizing a fall protection system. The Fred Olivieri Construction Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Duty to have fall protection), which was contained in OSHA inspection number 986482, citation number 01, item number 01 and was affirmed as a final order on 10/03/2014, with respect to a workplace located on East Maple St., N. Canton, Ohio 44720.
