29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:   On or about at 10/6/16 located at 5880 Vinyard LN, Cumming, GA: Employees were installing 2x4 rafters and plywood on to the apartment duplex that was under construction while on top of the roof approximately 28  foot from the lower level. Employees were passing up the lumber and the excess lumber was being dropped / thrown back down to the ground, employees were observed without hard hats exposing them to a struck by hazards.

29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used:   On or about at 10/6/16 located at 5880 Vinyard LN., Cumming, GA: Employees were using a damaged power cord on the skill saw with the insulation broken off exposing the wires. The cord had severe damage to the conductors.

On or about at 10/6/16 located at 5880 Vinyard LN, Cumming, GA: Employees were framing and installing the 2nd floor, roof rafters on an apartment duplex that was under construction while working at the edges of the staircase floor hole opening, the employer did not ensure that guardrails or floor hole coverings was used by the employees while exposed to fall hazards of approximately up to 10- feet.
