29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: On or about October 7, 2016, a ladder used by employees to access the roof was only extended past the upper landing surface approximately 17.5 inches. The ladder was not secured.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about October 7, 2016, during the removal and installation of roofing material on various levels of a roof with pitches of approximately 3:12, 7:12, and 10:12, the employer did not ensure employees were protected from a fall of greater than 12 feet by utilizing a guardrail system, fall protection system, and/or combination of warning line system and safety monitor system.
