OSH ACT of 1970 Section (5)(a)(1): OSH ACT of 1970 SECTION (5)(a)(1): The employer did not furnish employment and  a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards:    a) Pole #26: On October 8, 2016 and at times prior, employees were exposed to struck-by hazards while installing new power transmission lines from an Elliott bucket truck (Model: ECG-4-85-B Serial#2558 VIN#1HTSJAAR01H366281). The employer failed to ensure a thorough visual inspection was conducted to identify items such as, but not limited to, weld integrity on outrigger connections to the frame of the truck.     b) Pole #26: On October 8, 2016 and at times prior, employees were exposed to struck-by hazards while installing new power transmission lines from an Elliott bucket truck (Model: ECG-4-85-B Serial#2558 VIN#1HTSJAAR01H366281). The employer failed to ensure a thorough visual inspection was conducted to identify items such as, but not limited to, all outriggers, to include the front outrigger, were deployed.    d) Pole #26: On October 8, 2016 and at times prior, employees were exposed to struck-by hazards while installing new power transmission lines from an Elliott bucket truck (Model: ECG-4-85-B Serial#2558 VIN#1HTSJAAR01H366281). The employer failed to ensure a thorough visual inspection was conducted to identify items such as, but not limited to, damaged or deficient control levers being tied back on the control box.
