This most recently occurred on October 5, 2016 on the jobsite located at 8401 S. 33rd St, Lincoln, NE 68516. Employees were observed reroofing a residence on a 6 in 12 steep sloped roof at heights greater than 6 feet above the ground below.

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level: The employer is failing to ensure that all employees are protected from fall hazards when using a ladder to access and egress a residential roof. This most recently occurred on October 5, 2016 on the jobsite located at 8401 S. 33rd St, Lincoln, NE 68516 where the employer had employees using an extension ladder to re-roof the residential home. The ladder being used was placed on an uneven surface and not secured to the building. The uneven surface caused the ladder to lean to the left which could cause the ladder to shift and fall when used.
