29 CFR 1926.251(a)(1): Rigging equipment for material handling was not inspected prior to use on each shift and as necessary during its use to ensure that it was safe: On or about October 10, 2016, at 1291 Lake Hearn Dr. NE, Atlanta, Georgia: Employees performing rigging work were exposed to being struck by unstable loads of concrete forms. Inspections were not conducted on the rigging straps frequently and regularly to identify deficiencies in the rigging straps such as but not including mismatched length straps.

29 CFR 1926.1419(j): Anyone who becomes aware of a safety problem must alert the operator or signal person by giving the stop or emergency stop signal. (a) On or about October 10, 2016, the employee rigging the load, who was also acting as the signal person, failed to give the stop or emergency stop signal to alert the crane operator when the load being hoisted became unbalanced.
