29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure:   Instances including, but not limited to the following:  a) Job site, at/near 1612 Graham Court, Pittsburgh, PA: On or about October 11, 2016, during construction activities, the employer did not ensure that employees used fall protection systems/devices while up on top of building walls to mark joist locations where fall hazards of up to approximately 20 feet existed.   b) Job site, at/near 1612 Graham Court, Pittsburgh, PA: On or about October 11, 2016, during construction activities, the employer did not ensure that fall protection systems/devices were used near the tops of stairway openings where fall hazards of up to approximately 9 feet existed.
