Davidson Specialty Foods, Inc., On or about October 12, 2016, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2016: 1. On or about October 11, 2016, a temporary Warehouse Worker, working in the Credit Area of the Warehouse, sustained a broken ankle injury after the employee's foot was pinched between a pallet jack and wooden bumper. The injury met the general recording criteria in that it resulted in medical treatment beyond first aid. The injury was not included on the company's injury and illness log. The injury would have been appropriately captured with a check in Column J (Other Recordable Cases) and a check in Column M (Injury).
