On or about October 12, 2016, Warehouse, Overflow Area:  The door with a sign reading "Emergency Exit Only" was not provided with an illuminated "EXIT" sign.    B.  On or about October 12, 2016, Aisle 16 at the Tornado Shelter:  The illuminated "EXIT" sign above the exit door was blocked by the placement of palletized materials in location 625.

On or about October 12, 2016, Battery Charging Area:  A Genie GS-2632 Scissor Lift was parked within 26 inches of the face of the electrical panels in the area.  Panels included a 3-phase 120/208V GE electrical panel and a 3-phase 480V GE A-series panelboard, each with a working space of 36 inches.

Davidson Specialty Foods, Inc., On or about October 12, 2016, the employer did not record the following workplace injuries or illnesses on the OSHA Form 300 or equivalent for the calendar year 2016:  1.  On or about October 11, 2016, a temporary Warehouse Worker, working in the Credit Area of the Warehouse, sustained a broken ankle injury after the employee's foot was pinched between a pallet jack and wooden bumper.  The injury met the general recording criteria in that it resulted in medical treatment beyond first aid.  The injury was not included on the company's injury and illness log.  The injury would have been appropriately captured with a check in Column J (Other Recordable Cases) and a check in Column M (Injury).
