29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (a)  On or about October 11, 2016, and at times prior, employees were exposed to machine hazards associated with moving machine parts, point of operation and in-going nip points during die change and set up.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.      (1)  The employer failed to shut down or turn off the machine to perform servicing in accordance with their     established procedure, thereby exposing employees to amputation hazards while     they performed die change and set up during the normal production mode of operation.         (2)  The employer did not perform adequate lock-out/tag-out procedures for the tesker machine, as required in the     Lockout Procedures Tesker company manual, which states "lockout procedures are to be performed for die change      and set up".  This condition exposed the employees to amputation hazards.  Abatement Note:  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
