29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  On or about October 4, 2016, at the above addressed job site, employees were exposed to falls of up to 18 feet while performing roof construction while walking on steep roofs without a means of fall protection.

29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s)  On or about October 4, 2016, at the above addressed job site, the employer did not ensure that frequent and regular inspections of the worksite were conducted to ensure that employees had been using adequate fall protection systems while performing roofing construction as employees had slack lifelines and did not have sufficient number of anchorages.
