29 CFR 1910.132(a):  Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices and protective shields and barriers were not provided, used, and maintained in a sanitary and reliable condition  wherever it was necessary by reasons of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation, or physical contact:      (a) On and before 10/12/2016, at facility, for the cleaning crew working with corrosive cleaning chemicals such as, but not limited to, ALPHA HP bathroom disinfectant cleaner.  The employer did not provide personal protective equipment for the protection of extremities such as, but not limited to, faceshield, apron, and appropriate gloves.

29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment that would protect the affected employee(s) from the hazards identified in the hazard assessment:   (a) On and before 10/12/2016, at facility, for the cleaning crew working with corrosive cleaning chemicals such as, but not limited to, ALPHA HP bathroom disinfectant cleaner.  The employer did not assess and therefore did not enforce the use of personal protective equipment for the protection of extremities such as, but not limited to, faceshield, apron, and appropriate gloves.

The employer did not enforce the use of the personal protective equipment that was provided to the employees.

29 CFR 1910.151(c):  Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:     (a) On and before 10/12/2016, at facility, for the cleaning crew working with corrosive cleaning chemicals such as, but not limited to, ALPHA HP bathroom disinfectant cleaner.  An eye wash station was not immediately available for use in the event of eye contact.
