29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents. On or about October 4, 2016, at 1111 Gravier Street, New Orleans, LA., the employer failed to ensure that employees wore protective eye wear while using a Bosch Chipper hammer to chip down concrete from under a first story window.
