Librestone Marble & Granite, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.305(g)(1)(iv)(A), which was contained in OSHA inspection number 1098320, citation number 1, item number 4c, and was affirmed as a final order on April 28, 2016, with respect to a workplace located at 8470 E 86th Ave., Commerce City, CO 80025.

Abatement Note: The employer shall verify that the required workplace assessment has been performed through a written certification that shall include the following: 1) Identifies the workplace evaluated; 2) The person certifying that the evaluation has been performed; 3) The date(s) that the hazard assessment was performed; and 4) Which identifies the document as a certification of hazard assessment. Librestone Marble & Granite, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.132(d)(2), which was contained in OSHA inspection number 1098320, citation number 2, item number 1, and was affirmed as a final order on April 28, 2016, with respect to a workplace located at 8470 E 86th Ave., Commerce City, CO 80025.
