29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    a.   On or about October 5, 2016, at the NAS Jacksonville Commissary, the HA-11 lighting circuit was energized with bare conductor wire ends exposed from a junction box at the top edge of the meat cooler, exposing employees to a 277-volt electric shock hazard.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    b.   On or about October 5, 2016, the employer did not erect guard rails or provide anchor points for personal fall protection at the unprotected edge above the meat manager office, exposing electrician employees to a 10 feet fall hazard from the top of the cooler to the office below.

29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers:    a.   On or about October 5, 2016, at the NAS Jacksonville Commissary above the meat, produce, frozen foods, and dairy coolers and freezers, gaps as wide as 6 to 12 inches between the edges of the top of the coolers and the surrounding walls exposed employees to trip hazards in that covers had not been placed over the holes.
