29 CFR 1910.67(c)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about October 12, 2016, at job site location 125 Phelps Avenue, in Rockford, Illinois, an employee of Image Signs, Inc. was working from an aerial lift approximately 15 feet above the ground and was not required to use a personal fall arrest system. The employee was exposed to falling approximately 15 feet to the ground. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
